Champion Performance is operating via doxy.me for our patient portal. We will be emailing you a link to meet with your therapist once an appointment time is agreed upon. By submitting your request you are consenting to a virtual visit with a physical therapist licensed to practice in the state of Texas.
TeleHealth appointments will be billed through your insurance when applicable and may apply towards your yearly visit limit and total authorized visits. There is no guarantee that insurance will cover the cost of the visit. All balances not covered by insurance will be your responsibility to pay and will be adjusted to our TeleHealth cash pay rate listed below.
TeleHealth Cash Pay Rates:
New Patient Evaluation: $130
Established Patients and Follow-Up Visits: $100 for 1 hour
